Individual Support
About the Service
Fostering Independence, Autonomy, and Community Inclusion
At EMESON, we believe that true independence is built on a foundation of choice, self-determination, and tailored support. Our Individual Support service is a regulated, direct-care intervention designed to assist individuals with developmental disabilities in developing, retaining, or improving the essential skills necessary to live and thrive within their communities.
Rather than adopting a generalized care model, we focus on the unique profile of each individual. We work side-by-side with clients in their homes and local neighborhoods to ensure they can navigate daily life with maximum autonomy, dignity, and confidence. This service is structured around measurable goals, helping individuals transition from dependent care environments to active, self-directed community participation.
Person-Centered Daily Living and Skill Acquisition
Achieving personal independence requires practical, consistent skill development. Our trained support professionals provide targeted instruction and hands-on assistance across key areas of daily living, including:
Community Integration and Mobility: Building the confidence to safely navigate local transit, manage neighborhood orientation, and utilize community resources like libraries, banks, and recreational facilities.
Financial Literacy and Budgeting: Providing practical guidance on managing personal funds, understanding the value of currency, budgeting for weekly expenses, and learning safe banking practices.
Daily Living and Home Management: Supporting the acquisition of household skills, including meal planning, grocery shopping, healthy nutrition choices, and maintaining a structured, organized living environment.
Health and Wellness Advocacy: Assisting individuals in managing their medical and personal care schedules, understanding healthy lifestyle choices, and learning to communicate effectively with healthcare professionals.
Self-Advocacy and Personal Autonomy
A core objective of our Individual Support service is equipping clients with the tools to speak for themselves and make informed decisions about their lives. We emphasize personal rights and self-governance through:
Self-Directed Decision Making: Coaching individuals to identify their personal preferences, voice their choices clearly, and take an active role in planning their daily routines and long-term life goals.
Rights and Accommodation Training: Educating individuals on their rights within the community, ensuring they understand how to request necessary personal accommodations confidently and effectively.
Social and Interpersonal Communication: Developing meaningful relationship skills, understanding social boundaries, and building support networks that contribute to long-term social inclusion and emotional well-being.
