Community-Based Supports
About the Service
At EMESON, our definition of independence extends far beyond the four walls of a private residence. We believe that true self-sufficiency is only fully realized when an individual has the capability, the confidence, and the physical support necessary to access, navigate, and enjoy everything their local community has to offer. Our Community-Based Support program is not just a service; it is a specialized mission to eliminate the barriers—both visible and invisible—that too often keep individuals with developmental disabilities segregated from mainstream society.
For too long, individuals with intellectual and developmental disabilities (I/DD) have been defined by where they live rather than where they belong. Our program transitions the focus of care from a place-based model to a participation-based model. We create customized engagement roadmaps that empower our clients to reclaim their right to the public square, ensuring they can experience the rich social, educational, and recreational tapestry of their neighborhoods as equal and active citizens.
Comprehensive Mobility, Transit, and Environmental Literacy
We provide highly trained community support professionals who act as mobile habilitative mentors. This is not passive supervision; it is active, hands-on teaching designed to foster mastery over the community environment. We recognize that the ability to move freely is the first step toward self-advocacy and choice. Our staff works side-by-side with individuals to:
Travel & Mobility Mastery: We teach the fundamental and complex aspects of safe movement. This begins with pedestrian safety, geographical orientation, and landmark recognition. It scales to the confident, independent use of public transit systems, including learning to read complex transit maps, managing fare payment systems, utilizing real-time scheduling apps, and understanding transit etiquette.
Environmental Literacy: Different spaces have different unspoken rules. We help individuals develop the cognitive and social tools necessary to read these cues. We teach them how to interact respectfully and safely in environments ranging from a quiet municipal library or professional medical office to a high-sensory, chaotic retail mall or a bustling local festival. By fostering this literacy, we ensure our clients can attend town hall meetings, shop for groceries, manage banking transactions, and utilize public parks without fear of misinterpretation or social failure.
Cultivating Social Fluency and Safe Public Access
Independence in the community is heavily dependent on an individual’s ability to communicate their needs and manage social interactions safely. Our Community-Based Supports place a critical emphasis on social habilitation in real-world contexts. We help our clients build the social confidence to ask for directions, navigate transactions with retail clerks, order food at restaurants, and manage unexpected interactions with strangers.
Crucially, our program ensures that this movement is always forward-thinking. We work with individuals to build sustainable social networks within their neighborhoods. We connect them with local groups, inclusive events, and places of interest, ensuring that when our formal support is complete, they have a permanent circle of connection. Our goal is to make the individual a known and respected entity in their community—a neighbor, a regular customer, and a recognized face at community functions—breaking down the cycle of isolation and replacing it with the profound joy of belonging.
